To recieve physical therapy services, you will need a signed prescription for physical therapy from your doctor. Next, call the Peter A. Towne Physical Therapy clinic nearest you to set up an appointment. There are three convenient locations. Our office managers and staff will verify your insurance benefits and schedule you to see one of our licensed physical therapists.

Insurance Filing Information

We will help you by submitting all of your insurance claims.

We are pleased that you have selected us to treat you within the guidelines of our profession to help resolve current problems and hopefully, prevent further ones through the practice of physical or occupational therapy. Also, we will help you by submitting all claims to your primary, secondary and third party insurance carriers on a biweekly basis. In order to do this, we request all I.D. numbers, addresses, agent names and other pertinent information be supplied to us during your initial visit. You will receive a personal statement each month so that you can monitor your charges and your carrier’s participation. Some statements may not generate until your insurance company processes the claims.

If a co-pay is required by your insurance contract, payment will be due at the time of service. Other out-of-pocket responsibilities will be due with each month’s billing statement.
